ガーナ・イースタン州、アッパーマニャクロボ郡。道路の整備が十分でなく、広大なボルタ川（ボルタ湖とも呼ばれます）の両岸に人々が暮らす地域です。対岸の保健施設へ行くには、ボートで川を渡るしかない住民が多くいます。陣痛が来てから慌てて川を渡ろうとして、間に合わずボートの上で赤ちゃんが生まれてしまうこともあったそうです。

最寄りの保健施設まで10〜20キロ離れた地域も点在しており、ガーナの妊産婦死亡率は出生10万人あたり263人と、日本の約66倍にのぼります。出産にともなうリスクは、想像しにくいほど大きいのが現実です。

こうした状況を変えようと、ジョイセフは「コミュニティエンパワメントによる母子保健推進プロジェクト」をこの地域で進めてきました。対象となる住民は、郡の全人口にあたる72,160人。保健施設の建設や医療従事者の研修、住民への啓発活動を通じて、母子保健を地域ぐるみで支える仕組みづくりに取り組んできました。

その象徴が、アカテン保健センターに完成した、ガーナで初めての妊婦待機所「マタニティハウス」です。出産予定日のおよそ2週間前から滞在でき、最大10人を受け入れられます。川を渡るなど、出産のために長い距離を移動しなければならない女性も、安全な場所で出産の日を待てるようになったのです。

ボルタ川の対岸に暮らすクロク・セレスティーンさんも、この施設を利用したひとりです。これまで自宅や地元の小さな診療所での出産で、産後の出血などのトラブルも経験してきました。今回は予定日の2日前から川を渡って待機所に滞在し、夫や家族に支えられて女の子を出産。清潔で広々とした分娩室にとても喜び、「友人にもすすめたい」と話しました。

2026年5月には、プロジェクトの締めくくりとして、郡の中心地アセセワで成果を共有するワークショップが開かれ、保健スタッフやボランティアなど123人が集まりました。完成したばかりの妊婦待機所の運営ガイドは、アカテン保健センターの助産師さんの手に渡されました。

印象的だったのは、プロジェクトが終わったあとも自分たちで活動を続けていこうという、地域の人たちの強い意志です。郡保健局は施設の運営の仕組みを日常業務に組み込み、事業で培った仕組みやノウハウを郡内全体へ広げていく方針を示しました。ジョイセフと郡保健局との間で、この歩みを引き継ぎ発展させるための覚書も交わされました。

この3年間は、塩野義製薬株式会社のご支援によって実現したものです。ジョイセフは、こうした企業との協働をはじめ、さまざまな支援によって世界各地の現場を支えています。出産を控えた村の女性たちに、安心して「待つ場所」ができ、それを地域の人たちが受け継いでいく。こうした取り組みがこれからも生まれていくよう、Run With Heartを通じた皆さんの応援も、ジョイセフの活動全体を支える力になっています。

From a Rushed Crossing to a Safe Wait: Ghana's First Maternity Waiting Home

In Upper Manya Krobo District, in Ghana's Eastern Region, roads are scarce, and communities are scattered along both banks of the vast Volta River (also known as Lake Volta). For many residents living “over the bank,” the only way to reach a health center on the other side is by boat. Some women, racing to cross once labor began, ended up giving birth mid-river, before they could reach the other shore.

In some areas, the nearest facility is 10 to 20 kilometers away. Ghana's maternal mortality ratio stands at 263 per 100,000 live births — about 66 times that of Japan. The risks surrounding childbirth here are hard to picture from where most of us live.

To address these challenges, JOICFP carried out the project "Promotion of Maternal, Newborn and Child Health through Community Empowerment in Upper Manya Krobo District, Eastern Region, Ghana," supported by Shionogi & Co., Ltd., from June 2023 to May 2026. Reaching the district's full population of 72,160, the project built community-driven systems for maternal and child health — from constructing health facilities to training health workers and raising community awareness.

Among its lasting achievements is Ghana's first Maternity Waiting Home, completed at Akateng Health Centre in December 2023. Women can stay there for about two weeks before their due date, with space for up to ten at a time. Rather than rushing across the river once labor begins, women can now cross ahead of time and wait for the day of birth somewhere safe.

Kroku Celestine, who lives across the river, is one of the women who used the facility. Having previously given birth at home and at a small local clinic, and having experienced complications such as postpartum bleeding, this time she crossed the river two days before her due date and stayed at the maternity waiting home. Supported by her husband and family, she gave birth to a baby girl. Delighted with the clean, spacious delivery room, she said she would recommend it to her friends.

On 5 May 2026, marking the project's completion, a Dissemination Workshop was held in Asesewa, the district capital, bringing together approximately 120 participants — health staff, community health volunteers, and others who had supported the project on the ground. The newly completed Maternity Waiting Home Operations and Management Handbook was presented to stakeholders, including the midwife at Akateng Health Centre, who had contributed to its development. What stood out most was the strength of local commitment to carrying the work forward independently, even after the project's end. The District Health Directorate announced plans to continue using the monitoring tools introduced through the project and to extend activities to two additional sub-districts not originally covered. On 29 May, JOICFP and the District Health Directorate signed a Memorandum of Understanding to support the continuation and further development of these efforts.

These three years were made possible by the support of Shionogi & Co., Ltd. Alongside partnerships like this one, JOICFP's work around the world is sustained by many forms of support. For women who once raced across the river to give birth, a safe place to wait now exists — and local communities are carrying that work forward.
